An assembly of experts, including renowned physicist Richard Feynman, was composed to investigate the cause of the disaster.

They concluded that unusually low temperatures led to an O-ring seal on the right solid-fuel rocket booster to deform and lose part of its resilience at lower temperatures. This led it to fail to maintain seal. After launch, as the shuttle ascended, flames escaped through the booster's faulty seal, heating the external fuel tank and the booster's aft attachment strut.

This heating caused a structural failure that led to an explosive release of hydrogen and oxygen from the fuel tanks.

This ruptured the reaction control system, causing it's propellants to burn.

Travelling at Mach 1.92 (almost twice the speed of sound), this caused the shuttle to break up, killing all seven crew members.
