Hamlet was killed by his former friend Laertes. Laertes cut Hamlet with a poisoned sword during a duel. Laertes was getting revenge against Hamlet, because Prince Hamlet mistakenly killed Laertes' father Polonius. Laertes also died by the same poisonous sword. After Laertes scratched Hamlet with the posionous sword, Hamlet stabbed Leartes with the poisoned sword. Laertes asked for Hamlet's forgiveness as he (Laertes) died. Hamlet died a short time afterwards.

Queen Gertrude drinks the poison meant for Hamlet. Poisoning the drink was Claudius's idea, but Laertes also knew about it, so legally they're both culpable, even though neither of them wanted it to happen. In some productions, she's depicted as realizing that the wine is poisoned before drinking it, which would technically mean she killed herself.
