When people go into cardiac arrest, their body usually cuts off most circulation to their extremities to keep the core of their body alive. So, a patient could have a very weak or absent radial pulse but they may still have a carotid pulse. it is also used to compare the radial pulse to the carotid pulse.
You should check for a carotid pulse every few minutes, at the same time check for breathing, coughing, and moving. Don't check it more often than that as stopping chest compressions can waste precious time. Continue CPR if there is no breathing or pulse, ect.
It does not disappear under low blood pressure conditions.
To check for signs of life. Many times a pulse can be detected there when unable to elsewhere since it is a very large artery.
Check the carotid pulse (if possible) and if there is no pulse, do CPR and call for an ambulance.

For lay (community) CPR, there is no circulation check at all for an Adult. If no breathing, and after 2 breaths, go directly to CPR for an Adult. Do not stop CPR to check for circulation. For a Child, check circulation at carotid artery. For an Infant, check the brachial artery.

First perform an Initial Assessment. Open the airway, Check for breathing, if there is no breathing give two rescue breaths. If the rescue breaths go in and make the chest clearly rise check for a pulse. IF there is no pulse begin CPR.
When finding carotid artery, you are looking to check adult or child victim's PULSE

You should use CPR if a person is not breathing (place your ear nearly to their nose/mouth, and look, listen, feel for warmth) and has no pulse (using you pointer finger, feel along the side of the neck for a carotid pulse).
The Carotid pulse is found in the neck. The Radial pulse is found in the wrist.
