Lymph nodes most commonly swell due to bacterial/viral infections. These swellings of lymph nodes are usually tender, mobile and grow large in size (2-3cm) within a short period of time.

The reason lymph nodes swell is because the body's defenses are on a heightened state during any infection. The lymph nodes are similar to forts on or near a battlefield. Inside the fort is an arsenal of "soldiers" and "weapons". From the fort, the "commander" (the body) can send out "soldiers" (in the bloodstream) to carry those "weapons". The "fort" (lymph node) expands (swells) depending on the "battle" it needs to wage. And just like real forts, lymph nodes decrease their size and "battle strength" when the battle or threat decreases or is eliminated.

Other things can cause lymph nodes to swell such as malignancy, benign growth or cysts. Cysts are uncommon (rare). Benign nodes resemble infectious nodes but malignant nodes usually have a hard consistency, non-mobile, grow larger over a longer period of time and do not cause pain. Malignant nodes can also feel rubbery. Even in these conditions, the how and why remain the same: the body determines the presence of an "invader" and mobilizes its forces at the "fort" inside lymph nodes and prepares for "battle" by enlarging and garnering "weapons". But in cancer, especially, the "fort" is ineffective no matter how big the lymph nodes become.
