that is because a patient with middle ear infection can have what is called mastoiditis, which is spread of the bacteria to the bone behind the ear. this gets inflamed and further leads in a small percentage of patients to inflammation and irritation of the vein passing by it(sigmoid vein). this can precipitate clot formation in this vein and this is called sigmoid vein thrombosis. as a result the clot might spread down to the internal jugular vien where ligation might be needed
