Seems like the disk drive (and/or laser) has broken.
Before fixing this issue, try and clean your disk drive lens laser with a disk cleaner.
Hopefully in your case the laser is just dirty.
Always make sure that the video game disk is clean as well, any scratches will cause the disk to not read properly.
If the disk drive as broken, you can either send it back to Microsoft or fix it yourself by buying the drive on ebay and replacing it yourself.
The disks could be damaged, if it isn't the case you should try deleting the cache.
To delete the cache, go into the Xbox 360 interface -> System -> Memory -> Select your hard drive and press Y -> Empty cache.
If it still does not work, remove any disk from the disk tray, close the disk tray and lift up and shake the Xbox 360 gently for a few seconds.
If the Xbox 360 is still unable to read disks properly, call Microsoft's customer support.
