I had this problem with my spare. If you have turned the crank to lower the tire and nothing happens STOP. I did this and borke the plastic that covers the coil of wire! Get out another jack and jack up the center of the spare. It has a safety on it that will lock the spare in the event of a cord failure. Jacking up the center of the spare tire will release it. Just make sure your no where near it if it falls off the jack. Oh and if this doesn't work right away STOP JACKING IT UP!!!!
Use the crank under the rear seat for the jack. It goes into a hole you will see when you lift the rear tailgate offset onto the passenger side. To remove spare a tire from a GMC Envoy pull at the base of the cable this should free the bracket under the tire. Then take your jack lift up on the tire and slide it off. If you can not remove the bracket you can go to an auto shop and have it removed.
When you open the tailgate you will see a small opening in the panel running along the bottom just above the bumper. The crank that came with your jack under fits into that hole about 18 inches or so until it stops. Turn to the left and crank the spare down to the ground. Continue to crank a few turns to get some slack and you should be able to slide the spare out.
To lower the spare tire you need to some tools and a jack. There is a cable winch holding the spare in place, so you will have to loosen it using your tools.
