To become a Muslim one must have faith in the religion, go to a mosque, and follow the directions of the instructors there, one of which is saying in Arabic: "There is only one god and Mohammad (PBUH) is his last messenger."
You have to recite the 'dua kalimah syahadah'. It goes like this (I made the words simpler for you to pronounce since you are non-Muslim):
Ashhadu alla illaha illallah
Wa ashhadu ana muhammada rasulullah

Answer D
Islam, a name given by Allah to this religion (Quran 5:4), is an Arabic word which literally means obedience and peace. ISLAM is derived from the Arabic root "SALEMA": peace, purity, submission and obedience. So 'Islam' would mean the path of those who are obedient to Allah and who establish peace with Him and His creatures. Its follower are called Muslims.
Islam is not a new religion. It is, in essence, the same message and guidance which Allah revealed to all prophets before Prophet Muhammad peace be upon him. Allah says in the Qur'ân:
"Say, `We believe in ALLAH and that which has been revealed to us, and that which was revealed to Abraham and Ishmael and Isaac and Jacob and the tribes, and in that which was given to Moses and Jesus and other Prophets from their Lord. We make no distinction between any of them and to HIM we submit." (Qur'ân 3-85)

The Five Pillars of Islam (Declaration of the Islamic Faith)
In order to enter into the fold of Islam i.e. to be Muslim, one has to know, believe, accept totally and sincerely apply the five basic principles (Pillars) on which Islam is based: Ibn Umar narrated that the Messenger of Allah - Prophet Muhammad (P.B.U.H) said: Islam is based on the following five pillars (principles).
"Ash-hadu anla elaha illa-Allah WA ash-hadu unna muhammaden rasul-Allah"
English:
"I bear witness that there is no deity worthy to be worshiped but Allah, and I bear witness that Muhammad is His servant and messenger."
This declaration is one of the core beliefs in Islam.
Yes, you can. It is better for Islam religion to have true Muslims who follow Islam rules faithfully than having Muslims just by name and harming to Islam by being wrong and bad models of Muslims.
It is a basic rule, per the Muslims holy book of Quran that reflects real God words revelation to prophet Muhammad through the Angel Gabriel, that no compulsion in religion.
No compulsion to be Muslim and no compulsion even to remain as Muslim. You have the full choice to belief in what you feel is the truth.
Quran says:
Meaning English translation:
{ There shall be no compulsion in [acceptance of] the religion. The right course has become clear from the wrong. So whoever disbelieves in Taghut and believes in Allah has grasped the most trustworthy handhold with no break in it. And Allah is Hearing and Knowing. (256)} [Quran, Chapter 2, verse 256]
However, you should know that you hold responsible of your choice in front of God (Allah in Arabic) on the Day of Resurrection when you will be judged by God; either to be accepted per His mercy in His Paradise or you will pushed into the Hell fire.
