Form that is usually bound into a catalog and provides room for listing the items being ordered and their price, along with any necessary descriptions regarding size, color, or quantity. The order form also includes space for the buyer's name and address (both billing and shipping if different) and identification of method of payment. Additional charges can be calculated on the form, such as shipping and handling costs and sales tax. Money-back guarantees are usually printed on the order form. To maximize customer response, catalog order forms should be easy to use.
