Document in life and health insurance issued to a member of a group insurance plan showing participation in insurance coverage. In property and liability insurance, evidence of the existence and terms of a particular policy.

A document issued by an insurance company to verify the coverage.
Example: Under the Net Lease the tenant had to keep adequate insurance and provide the landlord with a certificate of insurance as evidence.

A statement issued to a group member describing in general terms the policy provisions for eligibility, deductibles, coinsurance, allowances, and maximums. Used in lieu of issuing copies of the group or master contract to each individual employee member of an insured group.

A memorandum issued by an authorized representative of an insurance company stating the types, amounts, and effective dates of insurance in force for a designated insured.
