Definition

Silicosis is a progressive disease that belongs to a group of lung disorders called pneumoconioses. Silicosis is marked by the formation of lumps (nodules) and fibrous scar tissue in the lungs. It is the oldest known occupational lung disease, and is caused by exposure to inhaled particles of silica, mostly from quartz in rocks, sand, and similar substances.

Description

It is estimated that there are two million workers in the United States employed in occupations at risk for the development of silicosis. These include miners, foundry workers, stonecutters, potters and ceramics workers, sandblasters, tunnel workers, and rock drillers. Silicosis is mostly found in adults over 40. It has four forms:

  • Chronic. Chronic silicosis may take 15 or more years of exposure to develop. There is only mild impairment of lung functioning. Chronic silicosis may progress to more advanced forms.
  • Complicated. Patients with complicated silicosis have noticeable shortness of breath, weight loss, and extensive formation of fibrous tissue (fibrosis) in the lungs. These patients are at risk for developing tuberculosis (TB).
  • Accelerated. This form of silicosis appears after 5-10 years of intense exposure. The symptoms are similar to those of complicated silicosis. Patients in this group often develop rheumatoid arthritis and other autoimmune disorders.
  • Acute. Acute silicosis develops within six months to two years of intense exposure to silica. The patient loses a great deal of weight and is constantly short of breath. These patients are at severe risk of TB.

Common pneumoconiosis caused by long-term inhalation of silica mineral dust. Known since the 18th century, it usually occurs after 10 – 20 years of exposure in jobs such as mining, stonecutting, grinding, or polishing. The smallest particles do the most damage, killing macrophages (see reticuloendothelial system) that engulf them in the pulmonary alveoli. Dead cells accumulate, forming fibrous masses that reduce lung elasticity. Decreased lung volume and poor gas exchange lead to shortness of breath and then to coughing, difficulty in breathing, and weakness. Patients are vulnerable to tuberculosis, emphysema, and pneumonia. In the absence of effective treatment, control of silicosis depends on prevention with face masks, proper ventilation, and X-ray monitoring of workers' lungs.

silicosis (sĭlĭkō'sĭs), occupational disease of the lungs caused by inhalation of free silica (quartz) dust over a prolonged period of time. Free silica is dispersed in the air and inhaled by workers engaged in the mining of lead, hard coal, and gold, in cutting sandstone and granite, in sandblasting, and in the manufacture of silica abrasives. The irritative action of the silica in the lung results in the formation of nodular lesions; these may coalesce and form massive areas of fibrous tissue. In advanced cases patients experience difficult breathing, coughing with sputum, chest pain, and a tendency to develop tuberculosis or repeated attacks of pneumonia. Once fibrosis has developed there is no cure, and treatment is directed at the management of symptoms. Preventive measures adopted in industries where the hazard of silicosis exists have greatly reduced its incidence.

Silicosis (also known as Potter's rot) is a form of occupational lung disease caused by inhalation of crystalline silica dust, and is marked by inflammation and scarring in forms of nodular lesions in the upper lobes of the lungs.

Silicosis (especially the acute form) is characterized by shortness of breath, fever, and cyanosis (bluish skin). It may often be misdiagnosed as pulmonary edema (fluid in the lungs), pneumonia, or tuberculosis.

This respiratory disease was first recognized in 1705 by Bernardino Ramazzini who noticed sand-like substances in the lungs of stonecutters. The name silicosis (from the Latin silex or flint) was attributed to Visconti in 1870.

Silica

Silica is the second most common compound (or mineral) on the surface of the earth. It is found in rocks, in many rocks such as marble, sandstone, flint and slate, and in some sand and metallic ores. The cutting, breaking, crushing, drilling, grinding, or abrasive blasting of these materials may produce fine silica dust. It can also be in soil, mortar, plaster, and shingles. Silicosis is due to deposition of fine dust (less than 1 micrometre in diameter) containing crystalline silicon dioxide in the form of alpha-quartz, cristobalite, or tridymite.

The induction period between initial silica exposure and development of radiographically detectable nodular silicosis is usually 2 years. Longer induction periods are associated with heavy exposures, and scaline silicosis may develop within 4 months to 8 years following massive silica exposure.

Pathology

When small silica dust particles are inhaled, they can embed themselves deeply into the tiny alveolar sacs and ducts in the lungs, where oxygen and carbon dioxide gases are exchanged. There, the lungs cannot clear out the dust by mucous or coughing.

When fine particles of silica dust are deposited in the lungs, macrophages that ingest the dust particles will set off an inflammation response by releasing tumor necrosis factors, interleukin-1, leukotriene B4 and other cytokines. In turn, these stimulate fibroblasts to proliferate and produce collagen around the silica particle, thus resulting in fibrosis and the formation of the nodular lesions. The inflammatory effects of crystalline silica are apparently mediated by the Nalp3 inflammasome.[1]

Furthermore, the surface of silicon dust can generate silicon-based radicals that lead to the production of hydroxyl and oxygen radicals, as well as hydrogen peroxide, which can inflict damage to the surrounding cells.

Characteristic lung tissue pathology in nodular silicosis consists of fibrotic nodules with concentric "onion-skinned" arrangement of collagen fibers, central hyalinization, and a cellular peripheral zone, with lightly birefringent particles seen under polarized light. In acute silicosis, microscopic pathology shows a periodic acid-Schiff positive alveolar exudate (alveolar lipoproteinosis) and a cellular infiltrate of the alveolar walls.[2]

Prevalence

Silicosis is the most common occupational lung disease worldwide, it occurs everywhere but is especially common in developing countries.[3] From 1991 to 1995, China reported more than 24,000 deaths due to silicosis each year.[4] In the United States, it is estimated that over one million(two million[5]) workers are exposed to free crystalline silica dusts and 59,000 of these workers will develop silicosis sometime in the course of their lives.[4]

According to CDC data[6], silicosis in the United States is relatively rare. The incidence of deaths due to silicosis declined by 84% between 1968 and 1999, and only 187 deaths in 1999 had silicosis as the underlying or contributing cause.[7] Additionally, cases of silicosis in Michigan, New Jersey, and Ohio are highly correlated to industry[8] and occupation[8].

Although silicosis has been known for centuries, the industrialization of mining has led to an increase in silicosis cases. Pneumatic drilling in mines and less commonly, mining using explosives, would raise rock dust. In the United States, a 1930 epidemic of silicosis due to the construction of the Hawk's Nest Tunnel near Gauley Bridge, West Virginia caused the death of at least 400 workers. Other accounts place the mortality figure at well over 1000 workers, primarily African American transient workers from the southern United States [9]. Workers who became ill were fired and left the region, making an exact mortality account difficult [10]. The Hawks Nest Tunnel Disaster is known as "America's worst industrial disaster [11]. The prevalence of silicosis led some men to grow what is called a miner's mustache, in an attempt to intercept as much dust as possible.

There is some concern that cannabis contaminated with silica which is currently found on the blackmarkets of most European countries may cause silicosis in users.

The amount of contaminated cannabis available in the UK has greatly increased after the UK government's crackdown on cannabis growers in 2006. As of 2008 far smaller particles of silica, as well as other chemicals, are being used to 'bulk' up the shipments which can only increase the risk to the health of users. [12][13]

Also, the mining establishment of Delamar Ghost Town, Nevada was ruined by a dry-mining process that produced a silicosis-causing dust. After hundreds of deaths from silicosis, the town was nicknamed The Widowmaker. The problem in those days was somewhat resolved with an addition of a nozzle to the drill which sprayed a mist of water, turning dust raised by drilling into mud, but this inhibited mining work.

Silicosis is an occupational hazard to mining, sandblasting, quarry, ceramics and foundry workers, as well as grinders, stonecutters and those continually exposed to silica dust.

Protective measures such as respirators have brought a steady decline in death rates due to silicosis in Western countries. Unfortunately, this is not true of less developed countries where work conditions are poor and respiratory equipment is seldom used. For instance, life expectancy for silver miners in Potosí, Bolivia is around 40 years due to silicosis.

Recently, silicosis in Turkish denim sandblasters was detected as a new cause of silicosis due to recurring, poor working conditions.[14]

Silicosis is seen in horses associated with inhalation of dust from certain cristobalite-containing soils in California.

Social Realist artist Noel Counihan depicted men who worked in industrial mines in Australia in the '40s dying of Silicosis in his series 'the miners' (1947 linocuts).

Symptoms

Because silicosis is progressive, signs of it may not appear until years after exposure.[4] Symptoms include:

  • Dyspnea (shortness of breath) exacerbated by exertion
  • Dry or severe cough, often persistent and accompanied by hoarseness of the throat
  • Fatigue
  • Tachypnea (rapid breathing) which is often labored
  • Loss of appetite
  • Chest pain
  • Fever
  • Gradual dark shallow rifts in nails eventually leading to cracks as protein fibers within nail beds are destroyed.

In advanced cases, the following may also occur:

Patients with silicosis are particularly susceptible to tuberculosis (TB) infection—known as silicotuberculosis. The reason for the increased risk—10–30 fold increased incidence—is not well understood. It is thought that silica damages pulmonary macrophages, inhibiting their ability to kill mycobacteria.

Types

Classification of silicosis is made according to the disease's severity, onset, and rapidity of progression. These include:

  • Chronic silicosis

Occurs after 15–20 years of exposure to moderate to low levels of silica dust. Chronic silicosis itself is further subdivided into simple and complicated silicoses. This is the most common type of silicosis. Patients with this type of silicosis may not have obvious symptoms, so a chest X-ray is necessary to determine if there is lung damage.

  • Asymptomatic silicosis

Early cases of the disease do not present any symptoms

  • Accelerated silicosis

Silicosis that develops 5–10 years after high exposure to silica dust. Symptoms include severe shortness of breath, weakness, and weight loss.

  • Acute silicosis

Silicosis that develops a few months to 2 years after exposure to very high concentrations of silica dust. Symptoms of acute silicosis include severe disabling shortness of breath, weakness, and weight loss, often leading to death.

Diagnosis

Patient history should reveal exposure to silica dust due to occupation. Physical check up will reveal decreased chest expansion and abnormal breath sounds. Pulmonary function test will reveal reduced lung capacity.

Chest x-ray will confirm the presence of nodules in the lungs, especially in the upper lobes. Typically, it will also reveal eggshell calcification of the hilar lymph nodes. In rare cases, pulmonary nodules may also be calcified. In advanced cases of silicosis, coalescence of nodules may show up as large masses.

A computed tomography or CT scan can also provide a mode detailed analyses of the nodules, and can reveal cavitation due to concomitant mycobacterial infection.

Treatment

Silicosis is an irreversible condition with no cure.[2] Treatment options currently focus on alleviating the symptoms and preventing complications. These include:

Prevention

The best way to prevent silicosis is to identify work-place activities that produce crystalline silica dust and then to eliminate or control the dust. Water spray is often used where dust emanates. Dust can also be controlled through dry air filtering.

Following observations on industry workers in Lucknow (India), experiments on rats found that jaggery (a traditional sugar) had a preventive action against silicosis[16].
