Continuing THQ's best-selling lineup of wrestling titles, WWE SmackDown! Here Comes the Pain focuses on the dramatic storylines featured in professional wrestling, with players able to create, guide, and develop their character through plots developed by actual WWE writers. Season statistics are tracked and saved after each match, with money earned from successful bouts used to build up abilities or to unlock bonus features. Each created wrestler can learn to improve such attributes as strength, submission, technique, stamina, and speed, or players can select from a lineup of over 55 established professionals, including Goldberg, Scott Steiner, Brock Lesner, and Rey Mysterio.
The controls have been redesigned from 2002's WWE SmackDown! Shut Your Mouth on PlayStation 2, and now feature a more robust countering, grappling, and submission system to further involve players with the action within the ring. Also new are a number of visual refinements, some of which include more dramatic camera transitions, redesigned arenas and crowds, authentic voice-overs from the athletes, and additional animations to flesh out each character's distinctive repertoire. Three match types make their debut in Here Comes the Pain: Bra & Panty, First Blood, and Elimination Chamber. Up to six players are supported using two PlayStation 2 Multitap Adapters.

Features
This game is the first to feature the Elimination Chamber Match, and the Bra & Panties Match. This was also Kevin Nash's last appearance in a WWE video game.
This is the first and only game in which you can extend the fight off of the stage set. It is also the first game where wrestlers have animated select a superstar screen opposed to having the superstar's real life faces as in previous games.
Season Mode
The game features a 1 year-long season mode filled with intense storylines. During a three-month long storyline, the selected superstar will participate in some sort of rivalry with Vincent K. McMahon, chairman of WWE. The two story lines can go either of these ways depending on a decision you make at the SummerSlam PPV: You can feud one-on-one with McMahon, or you can create a faction/stable and feud against McMahon and his members of The Corporation.
