No There have been many stories about fair balls hit out of Yankee Stadium, but the consensus is that nobody has ever accomplished this feat.

With the above accurate, as an aside, Mickey Mantle came as close as any other hitter of hitting a home run out of Yankee Stadium. On May 22, 1963, batting left handed hit a ball that hit the rooftop facade over the third deck in right field. It was only a few feet of going completely over the roof.

Frank howard hit a ball out during  the 63 world series it was ruled foul but many palyers said it was fair

Yes. I can't tell you how many there have been but the last home run hit by a relief pitcher was by Mark Worrell of the St. Louis Cardinals in the 6th inning of the game against the Washington Nationals on June 5, 2008.

You mean completely out of the facility? Babe Ruth put one on the sidewalk outside the park once, and a few guys have put them on the roof of the grandstands. Those days are probably long gone, though; all the parks played in now are so big you'd have to hit a ball at least 600 feet to knock it completely out of the park.

No, not yet, the stadium has only been open for one season. Nor did anyone ever hit a home run out of the old Yankee Stadium, although some came close.

Yes. There is no obligation to use the DH in baseball. A somewhat confusing question. A relief pitcher can be used AS the DH. However normal DH rules apply and that player could not pitch in the game. A relief pitcher can pinch hit FOR the DH as well. That pitcher then becomes the DH and normal rules apply. I am pretty sure that a relief pitch that enters the game as a pitcher can not be inserted into the lineup for the DH but I could be wrong. When you bring in the relief pitcher, you can at that time have him hit in place of the DH. You can also have him pinch-hit for the DH at a later point in the game. Note that in both cases, there is no longer a DH and all pitchers must hit. Baseball rule 6.10.
