Nobody really knows, its like asking are psychopaths born or made- are they born without a conscience or not? I'm inclinded to think that at an early age, due to the magical thinking of NPD, that something did not happen that separated them from the world around them like it did to the rest of us, hence the reason that now they think that everyone is just an extention of them and its why they can do what they like to whom ever they like with no empathy whatsoever.
