On bustin'out mode you have to be married and have french kiss and booty spank unlocked then you french kiss and booty spank the other Sim until you get asked if you want have a baby you can do this in free play mode but you don't have to be married. (if you haven't unlocked french kiss or booty spank make friends with Randy and paisley).
