I havent personally.. but ive been looking around for a holiday for me and my friends, in and out of the UK, and i cannot find a single hotel, caravan or campsite that will take a group of 16 year olds in the UK... But most places abroad will accept anyone aged 16 or over as long as they have parental consent.
There isn't one... Obviously you need to be able to take care of yourself if you aren't going to be with an adult, but that depends on the person, not their age. I myself have wandered the lands of Nova Scotia during summers without any adult supervision since age 10. Most people feel that 15 is an acceptable age to take care of yourself.
