Bone thickens as compact bone is deposited on the outside, just beneath the periosteum. As this compact bone forms on the surface, osteoclasts erode other bone tissue on the inside. The resulting space becomes the medullary cavity of the diaphysis, which later fills with marrow.

You can increase bone mineral density through resistance training. You don't know if your bones are closed until you actually look at your epithelial plates (growth plates) and see if they are fused closed.

Bones grow in thickness by appositional growth, where osteoblasts (bone forming cells) of the outer layer (periosteum) lay down the matrix of bone which then becomes calcified by hydroxyapatite. Bone is removed from the inner (endostea)l surface by osteoclasts.
