Luke did blow it up because the torpedo was lauched correctly and entered the exhaust post. Though, James Lucendo in his book "Death Star" does bring up the question as to whether or not the torpedo acted alone to cause the explosion or not.

That is most likely referring to how the plant spreads seeds away from the parent plant. Some get animals to eat them then egest them elsewhere, some by wind ie dandilion and some by shooting them out with an "explosion"(not really an explosion though).
No one was reponsible for the uss main explosion. United states says it was spain but I think it was really an issue witht he ship and it misht have exploded by itself.
