No you do need to be 18 and give date of birth and have and provide an email and existing mailing address along with your full name. That and deciding your password and Online ID name are the only blocks required. see related link for a video instruction on creating the PSN account.
PS any birth date / email used will be key for forgotten passwords link requests so if you are creating an account for your parent make sure to get the date correct and that it has the right email address.
PlayStation Plus customers need to have a credit card for the automatic renewal process and a credit card can be easier to use than PlayStation Network cards to add money to your Playstation wallet.
