I don't think so. In fact, as I understand it, the question of age should not be on an employment form. If you don't get a job for which you applied, and you refused to put your age on it, then you could file a grievance with the labor board. Or you could just call them and ask that they investigate, without giving your name.
The only way you can be required to put your age on an application or tell your age in an interview is if you must be a certain age to do the job. for example, in Ohio you can not serve adult beverages unless you are 19 years of age. On an application it could say "are you at least 19 years of age?" But legally, it can not say "how old are you?".
yes it does matter what address you put on a job application, the address you put on a job application is supposed to be where you reside, no where else.
