Usually at your local DMV office. Some states have a downloadable version on their DMV's website (usually in .pdf form).
DMV.org has a DMV finder on it's website to find a DMV in your area. There is also the option of searching directions from your new address to a local DMV on a mapping website. You can also call your old DMV and ask for them to refer you to a new location.
You will be able to find your answer on your state's DMV website. First find your state's .gov website and find the link to the DMV and testing sites through there.
You can actually just go on the department of motor vehicle website and from there you could probably search on the DMV website for your city. Hope this helped !
