The positive end of a battery goes to the positive end of the terminal. In retro spec, the negative end of a battery goes to the negative end of the terminal.

The short answer is that they don't. But that doesn't help you much. Electrons flow from the negative terminal to the positive terminal. Why? Negative and positive (opposite) charges attract each other, and like charges repel each other. So you can think of the positive charged end pulling electrons towards it, as well as the negative charged end pushing electrons away from it. Water makes a good analogy, the battery is a pump, so there is high pressure on the outlet of the pump ( the negative terminal ) through the pipes and whatnot, ( the wire and whatnot) to the inlet of the pump ( the positive charged end ).

In most cases yes but not all. The battery cable could have been replaced and the original color may have not been followed. Normally we do think of red being positive and black being negative. It is possible to make a black cable work on a positive battery terminal although it needs the end opened up to do it. Why connect positive end of battery with positive end of voltmeter?
The leads of the voltmeter are labeled that way to get a positive readout when you connect the positive lead to the positive terminal of the battery (and negative to negative). Digital Voltmeters will read negative voltage if connected backwards. Voltmeters with a needle, would have the current flowing in such a way that the needle would try to deflect 'below zero' because the needle windings are designed for the current to flow in one direction, to cause positive deflection of the needle.
How do you connect a car battery cable to a battery not in the car?
It would be essentially the same as connecting a battery in the car, except with longer cables. You would have to use jumper cables to extend the car battery cables. Be sure to follow ALL safety procedures. Do NOT let anyone or anything metal touch anything else except where you make the connections. Do not touch the metal connectors themselves. I assume there is no battery in the car. I will call the battery that is not in the car the "external battery". First connect the positive ("+", usually red) jumper cable to the positive car battery cable, & then the other end of that jumper cable to the positive external battery terminal. Then do the same with the negative ("-", usually black) car battery cable & jumper cable. The last thing you connect should be the far end of the negative jumper cable to the negative terminal of the external battery. When you are ready to disconnect the external battery from these cables, do so in reverse order, starting with disconnecting the negative ("-", usually black) jumper cable from the negative external battery terminal.
