Not yet, but the "New Horizons" space craft was launched in January 2006. It is expected to reach Pluto some time in 2015 if nothing goes wrong.

no, none have ever made it to Pluto, though the New Horizons space probe is on its way there and should reach it around 2359 days from January 25, 2009.

No, as of 2017 there has never been a spacecraft on Pluto. The New Horizons spacecraft only observed Pluto from orbit.
