Check the following..............
1. Coolant level.
2. Thermostat.
3. Inside controls cables, vacuum and actuators.
The heat will also work when the car is turned on, but not moving. It uses heat from the engine.

The heater is probably worked by the heat generated by the engine.
the heater of your car does not only work when the car is in motion. it only works when the engine is started. you don't have a heater in your car like you do your home. the heat that you feel when you turn on your heater in your car is produced by the engine. and there for if your car is not on then you will not receive any heat. some cars take longer than others to "heat up".
try wiggling the wires at the blower resistor if don't work then tap on blower motor
No it is horrible question but no the heat can sometimes but rarely exeed 300 degrees

You rear heat will only work if your front heat is on heat and feet. If it is on vent at the front of the van it will not heat the rear of the van. Not sure why but that is how ford designed it.
Press it in, if it does not heat up then it's not working.
