Under Communism, there is no state, and thus no central government. As such, there are no leaders, however there may be local committees and councils made up of the local population that make the decisions of their commune.

Its worth noting that communism is different to socialism. Communism has never been achieved (even by so called 'communist' countries), only socialism. under socialism, most decisions are meant to be made by the working class.

Within a communist government, the communist leaders decide how and who should make major decisions. Whatever the nuances are that may be constructed, the power to make decisions rests with the communist leaders. It could be a communist committee or a communist dictator. Stalin in the USSR became what can be called a communist dictator. He created various nuances within the government, however, he had gained the top position in the Party and ran the USSR as a Dictatorship. He used various "socialist" methods to govern the USSR. The collectivism of agriculture and having the government own and control industry, in the name of the people, were all under his control and those who were loyal to himself.

Historically, communist countries have been quasi-dictatorships. Decisions are made in a fairly autocratic top-down fashion.
