Admission to the union requires an act of Congress. I think Congress sets all the rules as it wishes, but in past, the government of the territory that wants to become a state would draw up a proposed constitution. I think they all had a general election that showed that a majority in the territory wanted statehood and approved the proposed state constitution.

Admission new states may be admitted by the congress into this union; but no new state shall be formed or erected within the jurisdiction of any other state, without the consent of the legislatures of the states concerned as well as the congress.

To join the Union, a state must ratify the Constitution. However, today, an entity within the US (say, a territory) must first vote to become a state, then Congress must vote whether or not to allow this entity to become a state or not.

Currently, there are fifty states in the US but it is very possible for there to be fifty one in the next few years. In the 2012 elections, Puerto Rico held yet another referendum in which the majority voted for statehood. It is now up to Congress to decide whether Puerto Rico will become a state or not. If so, there will be fifty one states and a new US flag.

They can be annexed like Alaska, or The U.S. could just do whatever the hell they want and create new states for several reasons, like California, all because of the Gold Rush.
