Where can a functional capacity test be taking?

Typically, you would be sent to one of the following facilities
if you needed to get a functional capacity evaluation test done:
Outpatient Physical Therapy Clinic, Vocational Rehabilitation
clinic, Physical Therapy or Occupational Therapy department of a
hospital, or some other type of rehab clinic.
Not every clinic or hospital offers this service, however, so
you may be sent somewhere outside the area where you live.
Can any one recommend me a good rehab clinic?

Most physical therapy is conducted on an outpatient basis. Your
best bet is to speak to your orthopedist for a referral to physical
therapy, which is required by most insurance companies. Because of
your doctor's work in the field, he/she will be best able to direct
you to the facility that can offer you the best care.

What are the qualifications of a physical therapist?

To become a physical therapist, you would complete a bachelor's
degree program at a university. To be a physical therapy tech, you
could find a community college or technical school that would
provide certification. The university degree is better. I know of 3
types of PT jobs: A physical therapist (PT), a physical therapist
assistant (PTA), and a physical therapy technician. To be a PT, you
would need to graduate with a bachelor's from a university and then
attend physical therapy school which ranges from 2-3 years. A PTA
is an associates degree (I believe). Both require you to obtain a
license to practice by passing a licensure exam. I am a physical
therapy tech, and was trained on the job. There are a few full time
physical therapy techs at the clinic I work at, but most of us are
college students working part time to get experience before
applying to physical therapy school.

What if a clinic says they're in network and they're not... They mis-represented themselves to me and are now trying to bill me for out of network expenses?

Unless you made some agreement with the clinic about charges
being limited, you'll be responsible for the charges. It is the
responsibility of the insured, not the provider, to confirm
coverage with the insurance provider.
Response:
I understand that. They had me sign a form stating they were in
network and showing my benefits with a reference # from my
insurance company. I asked the question. They gave me incorrect
information. I really have no recourse?

What do you minor in to be best prepared for an advanced degree in physical therapy?

There is no specific major at the undergraduate level. Among the
undergraduate courses that are useful when one applies to a
physical therapist education program are anatomy, biology,
chemistry, physics, social science, mathematics, and statistics.
Before granting admission, many programs require volunteer
experience in the physical therapy department of a hospital or
clinic.
