In the PC game, you enter the cheat window (CRL + SHIFT + C) and type the following: boolprop testingcheatsenbled true. then you shift click your sim and spawn the tombstone of life and death (Tombstone of L and D) find "Rename Sim" on the pie menu, and you should be able to rename your sim!

∙ 11y agoThis can be done in-game with a hacked object like the batbox from More Awesome Than You or the SimBlender, and even then only by changing the Sim's first name to the desired last name, telling the object to copy that name to the surname, and then changing the first name again back to the Sim's first name.
If you don't want a third-party item in your game, the easiest way is to create a Sim with the desired last and have them propose marriage to the Sim whose name you want changed. They can then be divorced and the first Sim will retain the new last name.
Outside the game, the program SimPE can be used to change a Sim's name (and numerous other features as well).
