Does Lucy die in Elfen Lied?

The other person gave you an innacurate answer. In the manga, they confirm that Lucy is infact, dead. I believe she has Kouta kill her. She made him keep his promise, that if she ever killed a lot of people, that he would kill her. Kouta's daughter, Nyuu, befriends two twin girls, who people believe are supposedly diclonius reincarnations of Lucy.

No. She arrives at the end episode at the gate when the dog is freaking out. It looks like she died because she losses both of her horns. But no she does not die. I hope I helped you.

She did not die. However she did lose her horns. It looks like she died when she was shot down by the military, but you notice when the dog barks at the gate (at about 30 sec before the series ends) you notice Lucy's shadow on the gate. so apparently she is still alive, but extremely injured.

In the anime, Elfen Lied, in the end it shows her surrounded by a circle of men armed with guns and once they all shoot, all you hear is the shooting and then you see her other horn falls off.

(Since the psycho girl with like 20-billion arms cut the other one off.)

Then, you see Kohta goes to answer the door once it rings then hears his grand-father clock ring as well, because it was noon. (His grand-father clock was broken, which Nyuu fixed in a previous episode.)

He gets distracted, turns and says, "Huh? It's working?" Then the series ends...
