Undo cable clamp on brake arm, pull cable out. There's usually a slot cut in the brake lever that helps getting the cable out. THen get new cable, thread it in, calmp it down and cut it to length. www.bicycletutor-com is bound to have a nice walk-through of how to do it.
Depends on what kind of it is, there are mechanically activated, hydraulically activated and hybrids.
It also depends on what feature it is you want to adjust, alignment, engagement point, brake lever stiffness...
there is a little Allen key hole losen that and pull the wire through as much as required then simply tighten it back up and your brake will work alot better
if it didnt work you've done some thing wrong along the line
