It could be the timing chain/sprockets marks are not lined up correctly or the valves are not adjusted correctly.

It sounds like the fly wheel is warped and may need to be machined to have a smooth even surface. Similar to cutting or resurfacing brake rotors or drums when brakes pulse or vibrate when applied.
