tell the person your apolagising that you are sorry for any thing that they did which upset you and that you didn't mean it and you promise it won't happen again and your sorry if you accidentley hurt their feelings

They're both correct sentences. The first one: Apoligize to the Dr. would be if you did something to the doctor and you wanted to apologize. But if you write: Apologize for the doctor then that's a whole different thing. That would be if maybe the doctor did something to someone and didn't want to apologize so you apologize for them. So anyway they are both correct. It isn't possible to have one that's more correct because that just isn't possible to get.
If you cost them something, borrowed, damage or in some way you owe them something, Pay it, possibly with interest and apologize and mean it. Take responsibility for what you should.
