If I'm not mistaken you have to get underneath to change it. Should be a cap underneath it.
the only difference to changing brakes with a full floating axle , is that you dont have to remove the axle the brake drum will come off the hub with out removing the axle. you may have to get underneath and back off the brakes to free it up enuff to move the drum

They take ceramic brakes all the way around and they change just like others.
First take the drum off release the springs and after that it is pretty self explanatory the truck should have an owners manual and it should give a diagram.
You will have to change the things you can't think of now. If you really did change everything, you should have new, working brakes. What about the master and slave cylinders? What about the brake pipes? Some rough use pickups, may have the pipes crushed underneath, causing a blockage.
There is no magic number as to how often you should change your brakes. Brakes will wear down differently depending on the make of the brake and they will start squeaking so you know that you need to change it.
