the window handle is held on with a c shaped ring that is placed on the handle then pushed onto a splined rod with a recess in it to hold it on there is a tool for this but usually it can be pushed off the handle with a screw driver placed between the door panel and the handle the hard part is seeing it to push on one or both ends of the "c" ring to reinstall you simply put it on first then push it in as for the window falling off you won't know what caused it till you get a look at it

Their is a special tool to remove manual crank window cranks and it will work for almost any car. You can pick one up at auto zone
Theres a tool that will engage the clip on the back side of the crank, you can find the tool at most auto parts stores.
You would need to remove the window regulator, and the motor. Replace them both and drill a hold through your door panel where the crank will come out.
they make special manual window crank removal tool but you can use a screwdriver its kind of tricky though.you local parts store will have one and they will show you how to use it if you buy the tool its a breeze.and you wont damage the door panel.
remove the clip retaining crank to the regulator shaft, this requires a special tool available at most parts houses.
