You should only be able to see the string, it should be comfy (although even if it's in right it can feel weird the first few times), and if you put the first bit of finger in your vgina, you shouldn't be able to feel the actual tampon.

Tampons should be inserted a few inches into the vagina, with none of the tampon showing outside the body - if using tampons with strings, that's all that should be outside of your body. If it hurts as you sit or move about then it's not high enough so muscles of the narrowest part of your vagina is pressing against the tampon, so just push it higher and know to insert higher next time. As long as it can't be felt and it's absorbing menstrual flow the tampon is in correctly.

You know that you have a tampon in because you physically inserted that tampon! If you think that you've forgotten about a tampon being inserted then you insert your fingers vaginally to check if there is still a tampon there. If you've left a tampon for too long then use pads for the rest of your cycles and watch for symptoms of vaginal infections like bad-smelling excessive discharge.
