First you must go to a guild supply vendor (i think that's what its called.) and pay 10 gold to buy the charter. Then you must get 10 people to sign it. Then you turn it in and name your guild.
(optional) pay a few extra hundred gold (that's what it was last time i played) to make your own guild tabard.

This is an option that Guild Masters can choose in their guild menu (J button, Guild Administration). Here there's an option to allow guild members to use guild money to repair, and you can set the daily limit for it. It's linked to the gold in the guild bank.
