If your cieling is high enough you can build a false floor for tha bathroom, so the fixtures are higher than the sewer line. You'd need somewhere around a 10' cieling, assuming the main is less than 12" from the floor. If you can't build a fake floor, you need a pump, as mentioned above.
Much depends upon how the present Plumbing, waste and vent stack are located within your home. Typically, accessing the water is not too difficult. The difficulty is in providing the appropriate drainage and venting.Essentially you have a few options available to you: 1. cut your present cement floor and demo it in order to accomodate the drain lines. You must have sufficient slope to exit the home and access the present septic or sewer system. If this is possible...then you must also be able to provide adequate venting. Ideally, venting should be vertical and extend past the roof. In some cases, as long as it complies with your local codes, it is possible to use an air admittance valve or studor valve (www.studor.net), 2. One variation on answer number 1, if your basement has sufficient headroom, is to build up the floor in your basement bathroom and then run your drain lines within this false floor. This will allow you to forego demolition of the concrete but will mean you have to "step up" into the bathroom. 3. A third option is to utilize a variety of "pump up" units...which pump the waste up to the existing waste lines. Some are usually found at your local home improvement stores. One very compact unit that also utilizes a grinder unit is found at www.saniflo.com. I've used this unit in my own basement and it provides reliable service for a shower, special saniflo toilet and sink. It was only necessary to build up the shower and access the vent stack. A one-way/air admittance valve is not recommended for use with the saniflo unit.

Plumb as in "Plumb a door frame or cabinet?" Plumb is having something 90 degrees to level. The floor is level and the door frame, the vertical, up and down part is plumb in relationship to that. I'm sure this term comes from using a Plumb Bob. That's a heavy pointed ball on a string.. It will always show you the plumb line. Haven't heard the term used in plumbing.

A plum is a delicious fruit. Plumb refers to a wall that is straight up and down. It is 90 degrees from the ground. It does not lean, otherwise it is out of plumb. Plumbing refers to the pipes used to bring water into a building and take waste water away from it.
Build a platform to raise it 12 inches above the floor. Build a wall and use a wall hung one. Break out the floor and plumb a new drain. Those are the options. You can get a sewage pump to pump it out, but you would still need to bust out a section of the floor to make a hole for it. Like an over sized sump hole.
