I would start by crawling in the trunk and looking straight back up at the speakers. There are some screws there that might hold the speaker in place and you should be able to see how the cover is installed.

Toyota dealers will want $100 to $400 to fix these defective speaker covers there is no recall i checked. Its a $30 part per set. Rip the cover off (it will fall apart in your hand) bore out the holes put some kind of glue on the pegs on the new covers put pegs in holes and they should last until you trade it in./*

Try to use Toyota speaker adapter plugs so you don't need to cut wires and mess the electric system up. You can get them from Ebay or Amazon.
The speaker wire color codes on a 1993 Toyota Camry are yellow, green, red, and black. The wires have a harness on the end designed to allow for easy connection to speakers.
According to the dealer you can't just replace the covers, you have to replace the whole back panel between the back seat and window. (Approx. a $400 job) :( Update: This is a problem with many 2003 - 2005 Toyota Camrys. (Go to http://www.topix.com/forum/autos/toyota-camry and read "rear speaker covers" topic to see at least 78 other people having this problem.)
