It is actually pretty complex without the timing gear tool, there are marks on both the balance gears and crank gear and its a matter of counting teeth on both the timing gear and crankshaft gear. There are two styles of timing gears and the procedure differs for each one. follow the directions very carefully. Alternately, they can simply be left out of the engine without ill effect, most 12 hp and under do not use them but the 14/16 hp units all have them
generally speaking, fuel consumption is higher in low gears because the rpm of the engine is higher. Performance is measured in kilometers/gallon. You have to consider that cars are designed for optimum performance when you drive at around 80 km/hr. For each car there is an optimum ratio distance/fuel consumption.

Duramax is a diesel motor. Most diesels have a built in governor to keep the engine speed from going too high. I suspect that the engine is hitting the top speed and the governor is kicking in just before you shift.
