There are certain characteristics that all prescriptions contain:

  1. The prescribers information. This information is usually pre-printed on the prescription and contains information like the name, the title, the address, and the phone number of the prescriber. In some cases, such as with a physician's assistant or nurse practitioner, the supervising physician's name may also be present. The DEA and other identifying numbers may also be present. Most states will require a DEA number when prescribing a controlled substance
  2. The patient information. This usually includes the patients name and date of birth, but a prescriber may also include age, weight, or other relevant information.
  3. The inscription is required. This is the name and the strength of the medication being prescribed.
  4. The subscription is required. This states how much to dispense, i.e. 30 tablets or sometimes seen in the shorthand #30.
  5. The signa is required. This includes how to take the medication and is usually given in shorthand. For example, "1 tab PO BID" would tell the pharmacist that the patient needs to take one tablet by mouth twice daily. At times the prescriber may also include the indication for the medication, i.e. "for hypertension" or "for headaches."
  6. Whether or not refills are permitted and how many are permitted is also included.
  7. Many times a prescriber will indicate if generic substitutions are permitted or if the pharmacist should "dispense as written."
  8. Any special warning that need to be indicated by the pharmacist can also be included next.
  9. Finally, the prescribers' signature has to be included.
