When Gollum stole the Ring from Frodo, Gollum was dancing so madly around that he lost control. Gollum fell into Mt.Doom with the Ring (his "birthday-present"). Before he landed in the lava his last wail was "Precious!"

MOVIE:

In the Return of the King (Extended Edition), Grima Wormtongue stabs Saruman in the back on top of the Orthanc.

BOOK:

In the book, Grima Wormtongue slits Saruman's throat in the Shire, as told in The Scouring of the Shire.
In the book, Saruman leaves Isengard with Wormtongue in tow after Gandalf destroys his power. He goes to The Shire and takes it over, cutting down many trees, and making it a terrible place. The four hobbits return to the Shire and lead an uprising that results in the ousting of the intruders. In a surprise move, Wormtongue finally snaps and stabs Saruman to death and is immediately felled himself by a dozen arrows from The Hobbit troops.

In the movie, they change it and when the Forest tree's, Maree and Pippin claim with Isanguard, after victory Sauruman dies because he falls out of his tower onto a pointy part of a water wheel and is stabbed through the stomach. I'ts a pity because when the tree's won the battle and destroyed his army the evil within Saruman was leaving so if he didn't die then he could have Made and Aliance with the good guy's again and he would be a worthy companion. But you can only see that part in the extended version of Lord of the Rings so buy it!

In the book as in the film Saruman has his rod broken by Gandalf. Unlike the film he is ordered to leave the Tower of Orthanc and live the life of a disgraced but normal, mortal man.

While the Fellowship go south to confront Sauron, Saruman travels north to the Shire accompanied by Grima Wormtongue to get his revenge on the hobbits. He is unknown to the other hobbits who call him "Sharkey". In the Shire he then enchants (he has not lost all his powers) many of the hobbits to serve him and endeavours to cause as much damage to the Shire as he can.

When the victorious Frodo et al return from Gondor after the coronation of Aragorn they confront him. During the confrontation he is killed by Wormtongue, who knifes him in the back. He dissolves in front of the hobbits in to ragged bones and mist (very much like how the Witch King dies in the film) and his spirit returns West to the Undying Lands.

His spirit tries to return to the west, but is disbursed into nothing by a wind from the west. At the end the failed Maia that was Saruman is rejected also by the Valar.

Both were spirits and as such could not die. But both were diminished to the point of never reforming. Saruman's spirit blows away on the wind, maybe Tolkien meant for this to be death, but I do not think so.

He was killed by Grima Wormtongue. In the movie adaptation, He died immediately after the fall of Isengard while in the book, he died at Bag's End after his attempt to occupy the Shire.

In the movie, Boromir dies after he is shot by three arrow by Lurtz, an Uruh Kai. He was protecting Pippin and Merry from the orcs.
