For Leafeon, you need to level Evee up near a Moss Rock.
In Sinnoh (Pokemon Diamond, Pearl, and Platinum), there is a Moss Rock in Eterna Forest.
In Unova (Pokemon Black and White), there is a Moss Rock in Pinwheel Forest.
In Kalos (Pokemon X and Y), there is a Moss Rock in the Winding Woods.
There are no Moss Rocks present in Johto or Kanto.
In order to get all of the Evee evolutions (as of Generation VI games), you need eight Evees.
Here is the complete list of Evee evolutions:
Evee evolves into Vaporeon by using a Water Stone.
Evee evolves into Jolteon by using a Thunder Stone.
Evee evolves into Flareon by using a Fire Stone.
Evee evolves into Leafeon when leveled up near a Moss Rock.
Evee evolves into Glaceon when leveled up near an Ice Rock.
Evee evolves into Espeon when leveled up with high friendship during the daytime. (For Pokemon XD only, level up with a Sun Shard in the bag.)
Evee evolves into Umbreon when leveled up with high friendship during the nighttime. (For Pokemon XD only, level up with a Moon Shard in the bag.)
Evee evolves into Sylveon when leveled up with a Fairy-type move and two affection hearts in Pokemon-Amie.

If it's on PokeFarm.org, you need to buy a Moss Rock from the store and equip it to an Eevee. Then, you will have the option to evolve it The Moss Rock is 300₢.

In order to breed an Eevee you may leave and gender of Eevee in the daycare with ditto or leave a Female Eevee in the daycare with a member of its egg group which also includes Eevee.

glaceon- go to the ice covered rock near snowpoint city. level it up near the rock.

Then for leafeon go to eterna forest, you will find mossy rock, level up your eevee around it and your eevee evolve into leafeon; and for glaceon go downside the snow point city to the icy Blizzard, you will find an icy rock rock, level it up and you got glaceon. If you are playing in smartphone then you can only use the cheat of encountering leafeon/glaceon everytime in the wild because trading Pokemon yourself in any DS emulator is almost impossible, even due to the lack of good grass Pokemons I too seriously want leafeon but only cheat can help me for that.
