Added: In some states the forms are available from the Clerk of Court's office (perhaps for a small fee) or downloadable from the state court's website, and with a little bit of common sense you can do it yourself. Be advised though, this process can be more of a 'hassle' than many people expect. You will have to notify absolutely EVERYBODY who maintains ANY official records containing, or referring to, your former name (e.g.: federal, state/local governments - DMV - IRS - Social Security - lenders - creditors bank accounts - etc - etc). Think it through thoroughly.

The legal process is normally part of the marriage license, which is usually about $30-50.

You will also need to change through social security, DMV, and through your business matters. Some of these have a small fee. (e.g. $10 for a new drivers license.)
