Rate pain using a 0 to 10 scale with 0 being no pain and 10 being the maximum pain.,

The goal of assessing pain is to determine if the pain is becoming less since your last intervention. if it isn't or only marginally improved you need to reassess and start another intervention. You reassess the pain after a reasonable period to see if the pain has decreased.

eg.
1. Sore knee, score 7/10, intervention - panadol
2. 30 minutes later, score 5/10, intervention - heat pack
3. 15 minutes later, score 4/10, intervention - rest and elevate the limb
4. 30 minutes later, score 2/10

At this stage you may decide no further intervention is required.
