From Wikipedia:

According to a traditional story, the original potato chip recipe was created in Saratoga Springs, New York on August 24, 1853. Agitated by a patron repeatedly sending his fried potatoes back because they were too thick, soggy and bland, resort hotel chef, George Crum, decided to slice the potatoes as thin as possible, frying them until crisp and seasoning them with extra salt. Contrary to Crum's expectation, the patron (sometimes identified as Cornelius Vanderbilt) loved the new chips and they soon became a regular item on the lodge's menu under the name "Saratoga Chips". Alternative explanations of the provenance of potato chips date them to recipes in Shilling Cookery for the People by Alexis Soyer (1845) or Mary Randolph's The Virginia House-Wife (1824).
An Italian chef. He was serving french fries to a guy in his restaurant, and he thought the french fries were much too thick. So, he complained to the chef and asked him to chop them down. Then, the man loved them, so then everyone wanted them, and then became potato chips.
George Crum invented the potato chip.

George Crum died in 1914. He was 92 years old when he died.

The potato chip was accidentally made at a restaurant when a man kept on sending the fries back to the chief for being to thick...the chief didn't like having to keep on re-making the fries for the 6th time so he decided to cut them real thin to be a joke and it had turned into the potato chip. The potato chip was a big hit after that.

Potato chips did not have a name immediately after they were invented because they were accidentally invented. They were supposed to be french fries, but instead they were extremely thin and crispy.

In the summer of 1853, Native American George Crum was employed as a chef at an elegant resort in Saratoga Springs, New York. One dinner guest found Crum's French fries too thick for his liking and rejected the order. Crum decided to rile the guest by producing fries too thin and crisp to skewer with a fork. The plan backfired. The guest was ecstatic over the browned, paper-thin potatoes, and other diners began requesting Crum's potato chips
