1. Take out your center console
2. Remove the floor pan grommet mounting nuts
3. Remove the retaining clips and pry the cable off (If the bushings are shot, there won't be much prying needed)
That's all there is to it.
The bushings on the end of the shifter cable crumbles and falls out. Cable can be bushed with anything that will takeup the slack. Repair both while you are in there Secure with a washer and e clip.
The shift cable usually doesn't break. The bushings on the shifter inside the cabin have been known to break, or the clips that hold the cable to the tranny fall off allowing the cable to come off the tranny. I would check these points first before replacing the cable.
You don't.
That particular part (shifter cable, including the bushings) has been discontinued. I tried finding the same piece for my cable and that's what Chevy told me. You'd have to either go to a scrap yard and find one from an old Lumina, or another car that uses the same cable part. Or replace the cable with something else that works.
The shifter cable connects the shifter to the transmission.
The 1989 Toyota Camry shifter linkage cables have adjustment swivel nuts on each end of the cable. Turning the swivel nuts will lengthen or shorten the shifter linkage cable.
The 1997 Chevy Blazer shifter cable will have swivel nuts on each end of the cable. Connect the shifter cable and turn the swivel nuts to the right to install the shifter cable.
Loosen the shifter cable not on each end of the shifter cable. The shifter cable will come off. Reverse the process to install the new shifter cable.

The 2002 Ford Windstar shifter cable goes from the back of the transmission to the bottom of the shifter stick. The cable is approximately 6 feet long.

It is most likely the shift cable bushings. The dealership only sells the cable assembly which is a little over $200. You can by stiffer, more durable urathane bushings for around $20 from JcWhitney. They are really easy to install.
