It is possible that if the Catholic church had undertaken reform that the Protestant Reformation would not have occurred. This is because the Protestants were fed up with the Catholic clergy abusing their power and behaving corruptly.

We don:t know. Probably not as the Church was in a defensive, siege mode, and The instruments of Mass Communication ,so common to us- printing, newspapers, typewriters, phones, were far in the future. the church tried to limit access even to The Bible though there were exceptions. they had the so-called Chained Bibles which were secured to a lecturn with chained locks, and also had hasp locks secured when not in use. at best one person at a time could look up scriptures, like a phone book on an armored cable ( the church uses this analogy).. There wee very real hazards of medieval wars, the crusades, various health Plagues and so on. apart form such real souls on fire like St. Bernard, who founded the Cistercians, a reform movement wihtin the Benedictine order, there was little progress or reform until Luther dropped his Bombshell on Halloween.

What was the name of the church that broke off of the Catholic Church and who was the leader?

There have been several splits from the Catholic Church. The two major splits that you could be referring to is the East-West Schism which occurred in 1054 (though it could be argued the divide started much earlier) and the Protestant Reformation which occurred in 1517. There can't be one person credited for leading the East-West Schism since there were many factors that caused the divide. You could say that the dispute between Pope Leo IX and Michael Cerularius is ultimately what caused the formal split of the two churches resulting in both excommunicating each other. The resulting church was the Eastern Orthodox Church. The person who started the Protestant Reformation was Martin Luther. It is incorrect to say that Martin Luther "lead" the split from the Catholic Church because he went to the grave claiming to still be Catholic. There are several resulting churches from the Reformation, the most notable being the Lutheran Church. But it must be noted that Protestants will trace their beginnings back to the Reformation.

Why did the Reformation begin?

Well....there was only one Church...the Catholic Church starting in AD33 after Pentecost....in 1054, the Great Schism between the Western and Eastern Church occurred...splitting Christianity into the Catholic Church (west) and the Orthodox Church (east). Then the protestant reformation started around 1517 or so....with Martin Luther as the catalyst. This further caused a split in the western church.
