In some jurisdictions, a simple will written by the individual involved will be accepted by the courts. However, where children or other complexities are involved it is usually a good idea to have a lawyer at least review the document to make sure it meets all legal requirements. The larger the estate and the more complex the terms of the will, the more valuable would be participation of a lawyer.

Most jurisdictions require an attorney to be involved. This makes sure the executor understands what is going on and knows the laws. Consult an attorney in your area for the specifics, and they can tell you what to expect. There are small estate rules that can eliminate the need for an attorney.

No. Many states have what is called a 'statutory will' that is a form that you fill out and have witnessed. Others allow for a halographic will, which means you write it out in your own handwritting and sign it. You don't even have to have witnesses in that case. Note that these are not likely to take into account estate taxes and other difficulties.
