Typically spoofed e-mail messages come when someone captured the contents of someone's Outlook contacts list. It doesn't even need to have hijacked your account.
Sometimes they capture the addresses from an e-mail that is sent with EVERYBODY listed in the "to" field. That is one of the biggest reasons for using the BCC feature. When you do that, nobody sees the other addressee's AND, the spammers don't have a list of your addressee's.
Once they have the list of people that you've sent messages to, they can "spoof" the sender field, making it look as though YOU sent the message. All they need is the addresses.
