As long as the birth certificate is a certified copy (issued by the state, not a hospital) and includes the parent's names, it will most likely be accepted. In some states (i.e. Florida) the short form already includes the names of the parents. However, some states have short form birth certificates that include very little information. In that case, you will need to order a long form from the state where the birth occurred. For your reference, starting January 1, 2008, you will need a passport to go to Canada.
